Major Criteria

  • positive blood cultures of typical organisms (staph, strep viridins, strep gallolyticus, HACEK, enterococci), echocardiogram with evidence of vegetation, single positive culture for coxiella burnetii.

Minor Criteria

  • predisposing heart condition, IVDA, Temp >38, vascular phenomena, immunologic phenomena, microbiologic evidence.

Diagnostic:

  • 2 Major
  • 1 Major, 3 Minor
  • 0 Major, 5 Minor

Treatment:

  • Usually able to guide by growth of organism, therapy is usually 4-6 weeks of IV abx.
  • Indications for surgery are heart failure, uncontrolled infection, prevention of embolism.
